The world No. 2 had been expected to defend the ATP Masters 1000 title he won in Cincinnati in 2025. He has not competed since withdrawing during the second round of the Barcelona Open in April 2026, and subsequently missed both the French Open and Wimbledon.
Cincinnati tournament director Bob Moran said Alcaraz was doing everything possible to return to tournament play. The 23-year-old had recently posted training footage, but his withdrawal means he will not use Cincinnati for competitive preparation.
The Winston-Salem Open, which starts on August 23, is the final ATP event before the US Open begins on August 30. Alcaraz had not indicated in the source reports whether he would enter that tournament.
Alcaraz won the 2025 US Open, his second title at Flushing Meadows after his 2022 triumph. He began 2026 by winning the Australian Open and becoming the youngest man to complete a career Grand Slam. His Cincinnati withdrawal was reported separately by the Guardian and Al Jazeera.
